Copayments, Coinsurance & Deductible

If you have a responsibility for some portion of the payment associated with your plan, we’ll be happy to collect the applicable copayments, coinsurance, and deductibles at the time of service. We currently accept payment in forms of Visa, Mastercard, Discover, America Express, Cash, and personal checks.

Insurance Authorizations/Referrals

If your insurance requires you to have a referral or authorization to be seen, please obtain prior to your appointment.

Patient Accounts/Billing

Please contact our office if you have any insurance or billing questions. You can pay any patient balances in our office or online.

We try to obtain your insurance benefits 48 hours in advance from your appointment. If you have any questions about your responsibility please contact your insurance company or our office before your appointment.

Our staff is available by phone to field your medical questions Monday through Thursday from 8:30 a.m. to 5:00 p.m. and Friday from 8:30 to noon. Calls are returned throughout the day in order of priority. If the nurse or staff member is unable to answer your questions, they will consult the physician and return your call as soon as possible. For prescription refills, please call your pharmacy as they can expedite this process by faxing a refill request to us, which will be faxed back the same day. Prescription refill requests should be made during office hours. Of course, a doctor is always on call, 24-7, for emergency needs.
